With patch 4.7, the buffs from The Proven Champion T5 armor set have been moved to runes that can be bought for 70 or 90 relics. This is good news, since the stats on that armor set is total crap. The runes can be combined with existing, useful armor sets instead. That way there'll be a meager increase in stats from doing T5.
There are two runes called Major Rune Stone of the Proven Champion: A blue one that gives rank 5 buffs and an epic one that gives rank 8 buffs. Rank 3 doesn't exist anymore, so it's removed from the tables below.
Anyway, collecting the armor set is pointless. I'll just keep this reference picture.
Buffs are up for 20 seconds. Cooldown on GoDS IV is 20 seconds and buffs proc on last hit, so effect over time is slightly less.

Buffs are up for 20 seconds, but Breach IV has 35 seconds cooldown with fully specced Improved Breach. Everything has to be multiplied by at least 20/35 to get the effect over time:
